Athletic Physicals for 2025-26 Being Accepted

The South Park Athletic Department is accepting athletic physicals for the 2025-26 school year. Any students participating in any school district sponsored sport in
grades 7 through 12, must have a physical completed. Physicals dated on or after May 1st, 2025 are accepted for the entire 2025-26 school year.
Below are the links to register your initial physical and papers for the physician to fill out. NO PAPER COPIES WILL BE ACCEPTED TO THE SCHOOL, and all physicals MUST BE REGISTERED ONLINE. If you have any questions please reach out to Athletic Director, Patrick Colligan at:
