Health Services

In compliance with applicable law and regulations, and Board-approved health and safety plans, the Board shall require that district students submit to health and dental examinations, screenings and health monitoring in order to protect the school community from the spread of communicable disease, to ensure that the student's participation in health, safety and physical education courses meets the student's individual needs and to ensure that the learning potential of each student is not lessened by a remediable physical disability.

Each student shall receive a comprehensive health examination upon original entry, in sixth grade, and in eleventh grade, conducted by the school physician.

Each student shall receive a comprehensive dental examination upon original entry, in third grade, and in seventh grade, conducted by the school dentist.

A private health and/or dental examination conducted at the parents’/guardians’ request and expense will be accepted in lieu of the school examination.

The district will accept reports of privately conducted physical and dental examinations completed within one (1) year prior to a student's entry into the grade where an exam is required.

Annual vision, height and weight assessments are also performed. Hearing screenings are completed in grades K-3, 7, and 11.

Policy 209

All medications including over the counter medications must be prescribed by a licensed prescriber.  The South Park Medication Administration and Consent Form must be completed by the prescriber and signed by a parent/guardian for all medications.
 
All medication should be bought to the nurse’s office, or the main office if the nurse is in another building, by the parent/guardian or by another adult designated by the parent/guardian.

All medication shall be stored in the original pharmacy-labeled container and kept in a locked cabinet designated for storage of medication. Medications that require refrigeration shall be stored and locked in a refrigerator designated only for medications. The district shall not store more than a thirty-day supply of an individual student’s medication.
 
Medication should be recorded and logged in with the date, name of student, name of medication, amount of medication, and signatures of the parent/guardian or designated adult delivering the medication and the school health personnel receiving the medication.
 
Nonprescription medication must be delivered in its original packaging and labeled with the student’s name.
 
Prescription medication shall be delivered in its original packaging and labeled with:

1. Name, address, telephone and federal IDEA (Drug Enforcement Agency) number of the pharmacy.
2. Student’s name
3. Directions for use (dosage, frequency and time of administration, route, special instructions).
4. Name of licensed prescriber.
5. Prescription serial number.
6. Date originally filled.
7. Name of medication and amount dispensed.

 All medications shall be accompanied by a completed Medication Administration Consent and Licensed Prescriber’s Medication Order Form, or other written communication from the licensed prescriber.

Policy 210

A child must have the required doses complete, or medically appropriate doses up to date and a plan in place for remaining doses by the fifth day of school, or they are at risk of exclusion. For a single dose vaccine, the child must have this vaccine before the first day of school or the child may be excluded from attending. Talk to your child's pediatrician about the vaccines your child needs to attend school.

• Four doses of DTaP (one dose on or after the 4th birthday)
• Four doses of polio (fourth dose on or after 4th birthday)*
• Two doses of measles, mumps, rubella (MMR)
• Three doses of hepatitis B
• Two doses of varicella (chickenpox) or evidence of immunity

Seventh grade:  In addition to those above, one dose of Tdap and one dose of MCV4
12th grade: In addition to those above, a second dose of MCV4

*A fourth dose is not necessary if the third dose was administered at age 4 or older, and at least 6 months after the previous dose
