Act 114 Clearance

FBI Background Check

To register, click here.
Our Service Code for registration is: 1KG6XN
You will need your registration number and photo identification at the time of your fingerprinting. Find approved fingerprinting locations by clicking here.
Your results will be sent to your email for a “one-time” opportunity to print the clearance. However, if you supply the registration ID# (it begins with “UE”) we can access the results online.

Act 168

Sexual Misconduct/Abuse Disclosure Release

Click here to download the Act 168 form. The candidate is to complete the top section about the current or former employer and Section 1 then submit to Jessica Dorsey in the Central Administration Office.  An Act 168 Form must be completed and submitted for ALL current employer(s) and to former employer(s) that were school entities and/or where the applicant had direct contact with children.

Act 126

Mandated Reporter Training

Complete the online, three-hour training by registering through the University of Pittsburgh by clicking here. After completing the training, save or print your certificate and email it to Jessica Dorsey (jessica.dorsey@sparksd.org).

Arrest/Conviction Report

Physical & TB Test

Click here to download Pennsylvania's School Personnel Health Record. A physical and TB test is required after an offer of employment has been made.

Drug Test

South Park School District policy requires a drug test. Contact Jessica Dorsey by calling (412) 655-3111, extension 1000 or email jessica.dorsey@sparksd.org to schedule your screening at a Quest Diagnostics closest to you. Do not go directly to Quest Diagnostics without scheduling an appointment with Mrs. Dorsey.
